CAMPER REGISTRATION FORM for leaders, assistant leaders, apprentice leaders and volunteers
**Please register no later than the 29th of May 2023**
Day Camp is a learning and adventure program run over 5 days 8.30am to 4pm during the June-July State School holidays (26th to 30th June 2023). It is for primary school age children (grades 1-6) It involves adventure activities, games, singing, craft activities, gunyah making, challenge activities and sports. Day Camp is held at Karingal Scout Camp at Mt Cotton.
Day Camp is also a way young people can be introduced to the love God has for them, through the sense of community that comes from experiencing a camping environment and appreciating the wonder of Gods creation.
For leaders, Day Camp is a way of growing in faith and developing leadership skills through practical experience. Two to three leaders are placed with a small team of no more than 10 campers, allowing for the development of good relationships, a supportive environment, and a fun experience.
Please note: The number of children who can attend camp is in proportion to 1 leader to every 6 campers. So, the more leaders the more campers we can take.
Camper Registration opens on 1 June, so we need to have all the leaders registered before 1 June, so we know how many campers we can take.
- All adult leaders must have a current working with Children Blue Card.
- All leaders are invited to the Leaders Pizza Party getting to know you on 16 June 6.30pm-8.30pm at Cleveland UC Hall. It is non-compulsory but helpful to attend.
- All leaders are required will do Day Camp leader training on the 25th June, 12:00-3:30pm at Karingal (camp location).
- All leaders are required to do the Safe Ministry with Children Day Camp training which will be sent to you once you are registered.
Leader Levels:
- Apprentice Leader (Grade 7 to 8)
- Assistant Leader (Grade 9 to 10/11)
- Leader (Grade 11/12 and Adult)
Registration for all leaders and volunteers is free. Please consider being a Leader.

Blue Cards are required for all people who are 18 and over. These are free and can be organised through the RUC - Cleveland Office. Referee Contact Details (FOR THOSE NEW TO Day Camp)
Referee information: as part of our commitment to providing a safe environment for participants and team, those providing leadership are asked to provide a referee. This should not be a member of your immediate family but can be someone who knows you like your pastor, youth leader, chaplain or a teacher from school.
8. Other family members who will be attending Camp
Remember to fill in a camper registration form for each family member.
9. Collection Details
You or an authorized person are responsible for collecting your child from day camp each afternoon.
All those under 18 must be signed out by an adult upon collection, but photo identification checks and approved lists of people who can sign participants out will only be enforced when a specific collection plan is requested from parents on this form.
If, due to custody issues or other reasons you require a specific collection plan for your participant, please indicate below and you will be contacted by the camp director to arrange an individual risk management plan relating to the collection of your child.
10. Authorisation/Consent
To be accepted by the registering leader/volunteer, (18 and over) or by their parent/guardian (Under 18).
For those 18 and over, these statements refer to you, for those under 18 the parent/guardian.
10.1 I confirm that the information provided in this document is true and correct.
10.2 I warrant that I have power to provide this information and agree to inform the Church of any change to these details.
10.3 I consent to (myself or my child) becoming a member of and taking part in the overall activities of the above mentioned program. *
10.4 I understand that if urgent medical attention is needed, every effort will be made to contact the parent and emergency contacts listed on this form. In the event that they cannot be contacted in an emergency, I give my permission for (me or my child) to receive medical treatment as deemed necessary.
10.5 I give my permission that my contact details may be used to inform me of other youth and families activities run by Redlands Uniting Church.
10.6 I consent to information about (me or my child) being collected and used for the running of activities and reporting of incidents and accidents.
10.7 I warrant that I will advise the activity leader of unique circumstances regarding (myself or my child) that may affect their participation in the activity.
10.8 I warrant that I will advise the Church of any change of circumstances that would affect the Church's care of (me or my child) or changes to the details of this form. *
10.9 I give my permission for (me or my child) to be transported in church vehicles or private cars arranged by the leaders on occasions when it is necessary.
Media Release
I give my permission for my child applicant or myself to be photographed, videotaped or recorded. I understand that this material (including name and congregation or activity) may be used and disclosed for the following purposes:
In promotional or information materials produced by the Church
In worship and materials prepared by or for the Church
On the Church's website and social media platforms
As otherwise permitted or required by law.
